Telehealth Informed Consent

Telehealth: Bringing compassionate care to your cozy corner.

IMPORTANT NOTICE: If you face emergency medical or mental health situations, PLEASE DIAL “911” IMMEDIATELY or head to the nearest emergency room. DO NOT use these services for urgent needs.

Telehealth Description: Telehealth utilizes secure two-way audio-visual communications, information technology, or other tools to enable healthcare professionals and patients in different locations to share patient-specific health details for providing clinical health care services.

1. Services Overview:

Precise Mind Behavioral Health, LLC provides various non-clinical administrative, financial support, and technology licensing services. This technology is employed by Precise Mind Behavioral Health, LLC and associated healthcare professionals (“Providers”). Although Precise Mind Behavioral Health, LLC provides this platform, they do not diagnose or treat any medical conditions or decide on healthcare service. Medical Groups and Providers might offer patient consultation, diagnosis, treatment recommendation, medication prescription, and other healthcare services through telehealth, based on the clinical appropriateness decided by the Provider.

2. Electronic Communications: Various electronic interactions can occur on this telehealth platform, including appointment scheduling, medical form exchanges, two-way interactive audio with store-forward communications, treatment recommendations, prescription reminders, and other transmission types.

3. Expected Benefits: Some potential benefits include improved care accessibility, lower costs, enhanced patient-Provider interaction, increased privacy, easier follow-up care, and efficient care management.

4. Service Limitations:

FOR ANY MEDICAL EMERGENCIES, PLEASE CALL 9-1-1 OR HEAD TO THE NEAREST EMERGENCY ROOM. IF FACING SUICIDAL THOUGHTS OR ACTIONS THAT MAY HARM YOU OR OTHERS, CONTACT THE NATIONAL SUICIDE PREVENTION HOTLINE AT 9-8-8 OR TEXT “HOME” TO 741-741.

Telehealth's main distinction from direct, in-person services is the lack of direct physical contact. Therefore, some clinical requirements might not suit telehealth, and your Provider will decide based on professional medical judgment.

5. Security Protocols: All services through this platform are facilitated over a secure connection in compliance with the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”).

6. Potential Risks: Risks might include delays in evaluation due to equipment failures, the inadequacy of transmitted information, breaches of personal medical information, and the requirement for in-person consultations in certain cases.

7. Patient Rights for Telehealth: As a patient, you have rights to refuse telehealth services, be informed of the Provider’s credentials, ask about the equipment and technology, and more.

8. Specific Informed Consent for Telehealth: By moving forward, you acknowledge understanding and agreeing to all outlined details.
